In order to register, please fill the following forms:

Cost:

JSKA UK / TOKON SHOTOKAN KARATE ACADEMY Membership is £40 per year. This runs from 1st April to 31st March of the following year. Those who join at any time after April will have this cost pro-rated at the cost of £3.5 per month (for example, 6 months from 1st October would be £21).

What do you get?

• JSKA UK Membership. JSKA UK is the UK representative body of the Japan Shotokan Karate Academy.
• JSKA JP Membership
• Tokon Shotokan Karate Academy Membership.
• Valid insurance to practice Shotokan Karate. This extends to competitions and tournaments.
• An official JSKA UK licence book, which also acts as a record of your progress.

Further Notes for Instructors

• All instructors must provide their own DBS (Disclosure and Barring Service) check.
• Instructor’s insurance is covered within the terms of the insurance obtained through membership of the Tokon Shotokan Karate Academy (since April 2021)

• PLEASE NOTE: If you have answered ‘YES’ to any of the above questions, you are advised to seek medical advice / approval before commencing any exercise session.
• In the event that medical clearance must be obtained before participation in an exercise I confirm that I have been advised to contact the GP and obtain advice (and, where appropriate, approval) prior to the commencement of the activity.
• I understand that ‘I am’ / ‘my child is’ (delete as appropriate)responsible for monitoring ‘myself’/’him or herself’ (delete as appropriate) throughout any activity, and should any unusual symptoms occur, would ease participation and inform the instructor.

Conditions of Registration Disclaimer

Definitions

• “Personal injury” has its ordinary English meaning and includes any injury for which person might be awarded under General Damages and/or Special Damages at Common Law.
• “The Releasors” means the family, dependants, heirs, executors and/or assigns and any person or persons claiming through them.
• “The Releasees” means any corporation, association or other body and/or any person(s) jointly where any such corporation, association, other body or individual is associated with the promotion or conduct of the lesson or Tournament and includes competitor(s), referee(s) and observers or members of the audience.
• “Any action” means any claim, right and/or course of action for damages at Common Law or pursuant to any statute.

1. I acknowledge that I have read the definitions set above.
2. I hereby acknowledge that my participation in lessons, entry and/or participation in a tournament carries with it a significant risk of personal injury.
3. Therefore, I for myself and the Releasors hereby relinquish, release and/or waive any action against the teachers and organisers for any personal injury sustained by me arising out of and/ or in the course of the lesson or Tournament whether as a result of any disregard and/or technical break of any rules of the lesson, event, tournament or otherwise.
4. In addition , in the event of any action being commenced , I for myself and the Releasors hereby indemnify the Releasees against any cost and damages arising from or connection therewith.
5. By training or entering any event I agree to be bound by the rules set by the JSKA UK / Tokon Shotokan Karate Academy.
6. I hereby authorise Jana Gee/ Kenneth Beuden or their nominated representative to capture photographic , digital or other electronic images during the course of any lessons or events. I further acknowledge that Jana Gee/ Kenneth Beuden own all Intellectual Property rights in respect these photographic, digital or other electronic images and may, at their sole discretion, publish, display or authorise use of these images for promotional purposes.
7. I consent to receiving First Aid treatment should the need arise.
8. I promise to pay the monthly fee on the 1st of each month and I agree to give one month notice before cancelling membership.
